Transitional Housing Program

Mountainlands Community Housing Trust has provided the Transitional Housing Program since 2003. This supportive homeless prevention program is the first of its kind in Summit County and currently provides 9 temporary housing units and support services for families and individuals in need. Eligible participants are provided with a place to live and are connected with available resources, case management and mentorship. MCHT’s commitment to these families and individuals is to create a bridge to independence, enhanced life skills, and affinity within our community.

Individuals are considered for the program through community referrals from partnering organizations and on a walk-in basis. Once qualified, participants create a service plan in order to achieve the three highly defined goals of the program: 1. Obtain and remain in permanent housing; 2. Improve self-determination; 3. Increase skills and/or income. Participants are encouraged to access mainstream resources, social services, financial literacy education, parenting programs, language and literacy programs, and volunteer opportunties in the community.

transitionalSince its inception, the program has experienced great success. To date, the program has served 79 families, consisting of 168 adults and children. Of the participants that left during the operating year 2008 – 2009, 80% successfully completed the program and had full time employment. 100% had a steady source of income upon leaving and entered either a rental assisted or other affordable rental unit.
